("LHCG") (NASDAQ: LHCG).  Under the terms of the acquisition agreement, the Company's shareholders will receive 0.9150 of a share of LHCG for each AFAM share they hold, representing consideration of $60.87 per AFAM share based on LHCG's November 16 trading price.
